Your first step is to fill out the form below.

Our staff will review the information, and be in touch via phone and/or e-mail.  Midwifery spots are not assigned on a first-come, first-served basis; instead, they are determined by a combination of factors, including your due date. As a result, it may take up to a month to get a response.  Unfortunately, we can’t accept every applicant, and you may be placed on a waitlist or not offered a spot.  Please continue routine prenatal care with a family doctor/nurse practitioner, or obstetrician until you have been contacted by us.  Family Care Midwives does not give clinical care or advice before your first appointment.
